HISTORY: Noni (Morinda citrifolia) is found in many parts of the tropics, including the Pacific Islands, Southeast Asia, India, and the Philippines. Ethnobotanical data is plentiful from these various regions regarding the uses of the fruit, leaves, and root of the plant. Such indigenous uses are many and varied, including internal and topical applications, as well as food use. Noni fruit was in the pharmacopoeia of the ancient Hawaiians. Noni is still used by the local population in Hawaii for diabetes, high blood pressure, cancer, and other chronic disorders, although no clinical data exists to support these uses.

SCIENTIFIC STUDIES: Currently, three published journal articles exist regarding the activity of Noni fruit. All are from the University of Hawaii School of Medicine, and relate to the immunomodulatory and anti-cancer activity of Noni fruit in the laboratory (see references below). These studies demonstrate that Noni fruit is responsible for significant immune system enhancement which accounts for its anti-cancer effect. The author of these studies became interested in Noni due to anecdotal evidence in Hawaii regarding the apparent success of traditional self-treatment of cancer and other disorders with Noni fruit. The same articles discuss the active compounds as being high molecular weight polysaccharides which are soluble in water, but not in alcohol.

Although many marketers of Noni products state that the active compound in Noni fruit is an alkaloid called xeronine or pro-xeronine, there is absolutely no scientific basis for this claim. These compounds do not exist within the realm of peer-reviewed scientific research; claims based on their existence are false.

3. It is suggested that Noni PPT be used in clinical applications as a supplemental agent to chemotherapy for cancer treatment. Introduction A number of immune system stimulating polysaccharides have been found in plants, and some have been shown to have anti-tumor activity. The immune system has several different mechanisms for acting as a defensive shield against invading cells (bacteria, cancer, etc.), and the polysaccharides in these plants work by activating a number of these mechanisms.

Noni has been used in Hawaii and other parts of the Pacific as a remedy for numerous ailments, and this paper reports on the biological activity of this fruit. Materials and Methods Noni fruits were allowed to ripen and age until the juice seeped out of the fruit. A water soluble precipitate (solid material) called Noni PPT was produced from this juice. The Noni PPT was examined by various analytical methods and found to be a high molecular weight polysaccharide made up of four types of sugars hooked together (hence the name, poly-sacchar-ide ? many sugars). The tumor cells in this study were from a standardized clone of cancer which is used in animal studies, in this case, mice. Various immune system cells that defend the body against disease were isolated from different tissues and were used to determine if Noni PPT had a stimulating effect on their activities. Results and Discussion                                                                                       1. Noni juice or Noni PPT was injected into mice with active tumors. Both Noni juice and Noni PPT showed effective anti-tumor activity and the juice was slightly more effective than the Noni PPT. The juice treatments prolonged the life span of the mice by more than 75%. Effective dosages were 6-15 mg of Noni juice per mouse or 0.8 mg of Noni PPT per mouse every day or every other day for 4-5 days. The alcohol soluble part of the Noni juice had no effect.                                                                         2. Noni PPT provided significant anti-tumor activity by stimulating immune system cells to attack the tumor, rather than by acting directly on the cancerous tissue.                                                                                    3. Noni PPT works by activating a number of different mechanisms of the immune system, causing different types of immune cells to work together in limiting the tumor.                                                                                4. Noni PPT worked well with conventional cancer chemotherapy. When Noni PPT was used with smaller than usual doses of the chemotherapy agent, the life span and cure rates of the animals increased as compared with a normal dosage of chemotherapy alone. It is suggested that using Noni PPT with smaller amounts of chemotherapy agents in humans would improve the cure rate while at the same time reducing the severe side effects with chemotherapy.

ABSTRACT: The fruit juice of Morinda citrifolia (noni) contains a polysaccharide-rich substance (noni-ppt) with anti-tumor activity in the Lewis lung (LLC) peritoneal carcinomatosis model. Therapeutic administration of noni-ppt significantly enhanced the duration of survival of inbred syngenic LLC tumor-bearing mice. It did not exert significant cytotoxic effects in an adapted culture of LLC cells, LLC1, but could activate peritoneal exudate cells (PEC) to impart profound toxicity when co-cultured with the tumor cells. This suggested the possibility that noni-ppt may suppress tumor growth through activation of the host immune system.

Concomitant treatment with the immunosuppressive agent, 2-chloroadenosine (C1-Ade) or cyclosporin (cys-A) diminished its activity, thereby substantiating an immunomodulatory mechanism. Noni-ppt was also capable of stimulating the release of several mediators from murine effector cells, including tumor necrosis factor-alpha (TNF-alpha), interleuekin-1beta (IL-1beta), IL-10, IL-12 p70, interferon-gamma (IFN-gamma) and nitric oxide (NO), but had no effect on IL-2 and suppressed IL-4 release. Improved survival time and curative effects occurred when noni-ppt was combined with sub-optimal doses of the standard chemotherapeutic agents, adriamycin(Adria), cisplatin (CDDP), 5-fluorouracil (5-Fu), and vincristine (VCR), suggesting important clinical applicati
